What about a text captcha? Sort of like those authorization questions some people put on their IM clients to block spam, for example "Please type out a thousand and eleven in numbers".

Not sure about non-English users though....

I don't see the problem with my suggestion. Only those using a text browser have to insert 4 additional digits to a text filed when submitting a form. If you are using a css capable browser you wont even notice the captcha. So its not really a captcha.

I even could simplify it in a way that this "captcha" is disabled when you're logged in. Of course I need to add it to the registration form then.
