Big noise - insane volume increase after random time spent on YouTube.

After some time of watching videos on YT via Firefox, doing something else in other program and returning to FF, the sound level gets very nasty, past the clipping-level. It's very loud and noisy. It's impossible to recover from that - the only solution is to reboot.

I tracked this bug to this log fragment:

This started to happen after recent upgrade of ALSA and kernel. Before that FlashPlayer used to crash in FF but without messing with my sound.

EDIT:
I forgot to mention that the noise is getting worse over time. After two hours it's impossible to hear anything even after turning everything next to mute.

EDIT2:
Another update. Problem reocurred after rebooting and playing only one song in VLC. This time without any visible cause except the segfault of blueman_app:
