USB ports randomly dies..

On my Dell Precision M4500 laptop im having the issue that the USB ports randomly dies, the only way to use again the USB ports its to reboot the system, when this happens, dmesg shows:

[53929.040749] ehci_hcd 0000:00:1d.0: fatal error
[53929.044636] ehci_hcd 0000:00:1d.0: HC died; cleaning up
[53929.047385] ehci_hcd 0000:00:1d.0: force halt; handshake ffffc90000c2c024 00004000 00004000 -> -110
[53929.047386] ehci_hcd 0000:00:1d.0: HC died; cleaning up

After this, nothing that gets plugged on the USB port works..

When plugging other devices after the fault, does dmesg say anything?
Your output appears to be a disk drive.  Is it a thumbrive, or is it a rotating disk?  In other words, how much current does it draw?

Re: USB ports randomly dies..

I have the same problem. I noticed that mine happens mostly when it comes out of idling. It's no acpi stuff, just video.

Since my keyboard and mouse are both usb, they stop working as well. Hum... now that I checked all that's connected via usb, which is:
my keyboard, mouse, external hd (with its own power supply), speakers supply (turned off), idle webcam and the monitor usb-hub extension!

When the video comes back from standby, it's probably messing up my usbs!?! Is that possible? That sucks, how would I fix that?

Maybe I connected it wrong (is that even possible?) well, when it's working, I use the monitor usb ports normally with pendrives. (It's connected with a regular USB A-B cable).
I guess I'll just disconnect my monitor usb for now :-/

By the way, even though I got no keyboard and mouse, I can login remotely via ssh and get it to work again by removing and re-adding ehci_hcd.

rmmod ehci_hcd
modprobe ehci_hcd

[edit]
By the way, although my computer is not a Dell, the monitor is. Do you have (and use) your Dell monitor usbs @bzImage?

Re: USB ports randomly dies..

I have the same problem for months ago, now. I haven't found anything about this, I tried some forums but nobody couldn't help, so I decidet to try here (my English is not so good), and I found this topic. The problem appears on my girlfriend's notebook (with Arch Linux, of course), too.

Sometimes if I try to connect something via USB, it's just not working, but the already connected hardvares have no problem until they're plugged in, except the Bluetooth adapter. Have to reboot to make everything work, but the problem occurs many times a day randomly.

I have no error message in any log and the lsusb says that the connected devices are the ones that were connected before the problem occured, regardless of what's plugged in and what's not.

Here is my everything.log, the output of dmesg | grep usb and lshw.

What, so everyone's supposed to sleep every single night now? You realize that nighttime makes up half of all time?

Re: USB ports randomly dies..

@freyr you problem seems significantly different from ours. In yours, most your USBs keep working and you got no error log. It's unlikely that they're related.

This is a Dell precision M4500 laptop.. so i don't have a monitor, well i do.. but it don't have usb ports.. well it does.. but they are not in the monitor itself.. well they are but not directly attached to the laptop monitor..
