Applications turn headphone mixer to 100%

Some applications cmus, vlc and mplayer turn my headphone mixer to 100 %, no other mixer is changed. I haven't fully discerned if it's starting those applications or starting to play, but it seems the latter is more likely. This is annoying and more importantly damaging to my ears. I have no ~/.asoundrc and /etc/asound.conf is vanilla. As long as pulse doesn't have a curses mixer it's of no use to me, so it's not a solution.

How do I debug this? What could be a cause and how would I fix it.
