PulseAudio Volume Inconsistency

I constantly find myself having to manually adjust the volume using pavucontrol, because the volume will not stay where I left it. At random (it seems), the volume for either a device or just a particular stream will go down or up in volume. It won't happen while it's running live, but it will change on new instances on occasion.

For example, I might be watching YouTube. I set the stream's volume to 100% and use hardware controls (volume wheel on speakers, etc.) to control actual output. All is fine, and I finish. Hours later, I come back, and the stream is now at 159% (the maximum allowed). I changed no volume settings, but the sound gets significantly distorted at that level.

This happens to a friend of mine as well, where certain devices will get set to 0% volume at random.

What could cause this behavior? Is there a way to lock all channels at 100%?

There is a newer pulseaudio package in [testing] with many bug fixes. You could try that version (4.99.4-1).
