I just switched to a dual screen set-up, both monitors are connected to an Nvidia card (one using DisplayPort, the other one using HDMI). There's also a regular 3.5mm audio cable between the main monitor (DisplayPort) and the computer. Before, I used the main monitor using a HDMI cable (no second monitor).

The main problem turned out to be a setting on the main monitor (the audio input was set to DisplayPort instead of Line-In). However, this doesn't explain the two Headphone channels in alsamixer.
