Clean install of KDE5. The audio devices are sorta detected :
In "Sound card" there is :
- Built-in Audio
In "Profile" there are :
- Analog Stereo Duplex
- Analog Stereo Output
- Digital Stereo (IEC958) Output
- Digital Stereo (IEC958) Output + Analog Stereo Input
- Analog Stereo Input
- Off
When "Analog Stereo Duplex" is selected then in "Sound device" there are :
- Playback (Built-in Audio Analog Stereo)
- Recording (Built-in Audio Analog Stereo)
When other profile is selected then only either Playback or Recording option is displayed respectively
In "Connector" there are :
- Front Microphone
- Rear Microphone
- Line in
The microphone is basic 3.5 mm plug microphone connected to the rear port. When i choose Rear Microphone, the volume level from the microphone is visible in the bar in "Input Levels" : I tap the microphone or talk to it and teh bar jumps respectively
Now, in the Device Preference there is simply no device for the micrphone, in none of the "Audio Recording" tree branches :
At the same time, the device for the speakers is there and sound output from the speakers works fine :
One thing i noted tho, in KDE4 there used to be a list of ~4-5 devices for each component, like "Default", and devices listed by hardware address and by the card name/model, and so on. Here there is only "Default", this appears sorta too empty..
I tried gstreamer and vlc backends this does not make any difference :

Do you have gst-plugins-good installed? Is pulseaudio installed and running? (I'd guess so the tab that works suggests as much) For phonon to show the devices with the gstreamer plugin gst-plugins-good has to be there for the pulse output. You might also want to post
aplay -l
sudo fuser -v /dev/snd/*
and make sure nothing is blocking pulse's access (pulseaudio should be the only thing accessing your hardware)

I have a similar problem, i got "default" instead of my sounds card, everything works though but i can't choose which sound card to use in kmix, ditto with microphones but both sound card and microphone works on teamspeak and such.
edit: installing gst-plugins-good resolved my problem. I'm sorry for the post.
