Audio not playing through any devices although Pulse sees them

I had a working pulseaudio/ALSA setup leading to an external USB soundcard working on my computer for several months, but after I disconnected and reconnected the USB port leading to the soundcard, audio will not play.
Both ALSA and Pulse see the soundcard, and pavucontrol shows the audio that is supposedly playing through the card, but no sound is coming out of the speakers.
I've checked to make sure it isn't muted in ALSA or Pulse to no avail and reinstalled Pulse. I've tried switching the USB to other ports on the computer and bypassing the USB soundcard and connecting the amp directly to a 3.5mm out from the computer with no luck. I did notice through pavucontrol that Pulse did not correctly register that the line out being plugged in, claiming that line out was unplugged and headphones were plugged in, the same as when no devices are connected to any 3.5mm ports.

I noticed with Pulse that it plays speaker-test as mono whereas it is in fact a stereo card.

That all looks correct (run speaker-test with -c2 to create a stereo speaker-test) do you get any messages in dmesg, or the journal (use

journalctl --user-unit=pulseaudio.service

regarding your soundcard? other sanity checks, do you have some external control that might mute the soundcard on a hardware level or are you sure that your amp isn't actually turned down via hardware knob or similar, the cords are plugged in correctly?

No external mute for the soundcard, I've tried plugging the USB soundcard into a laptop and audio works fine from there.
Dmesg looks fine, but I got this error in journalctl:

hmm.. POLLNVAL might stem from file descriptor issues, is anything else blocking your card?

sudo fuser -v /dev/snd/*

should help out here. Only pulseaudio opened as your user should have your cards open. Another suggestion I have is to try and rule out a kernel regression by trying the lts kernel for example. Do you use any non standard pulse configuration? You might want to post your /etc/pulse/default.pa if that is the case.
