In gentoo I had tv out from my nvidia card working fine using nvtv. I've just installed nvtv from AUR but when I try to run it up I get the following message:
Fatal: No supported video card found.
The kernel module is loaded:
lsmod | grep nvid
nvidia 4528596 12
agpgart 26980 2 amd64_agp,nvidia
i2c_core 17408 2 i2c_viapro,nvidia
I'm using the same video card, same xorg.conf. I am starting the nvtvd as root before starting X.
All I can think of that is different is that with gentoo I got the kernel module directly from nvidia but arch comes with one already built as a module with the kernel.
Can anyone suggest anything to try?

What is your video card?I'm with gf2 mx400 and i haven't problems with nvtv.
EDIT:Have you started nvtv as root ?

The card is:
lspci | grep -i nv
01:00.0 VGA compatible controller: nVidia Corporation GeForce 6200 (rev a1)
I've started nvtvd as root then tried nvtv as root and normal user, both with same result. nvtv should be normal user interacting with the daemon running as root I think.
I've also just tried installing the latest version of the nvidia kernel module from nvidia themselves with no luck.

This will not solve the problem you have with nvtv, but why don't you use the tvout functionality of the nvidia xorg driver? Here's the xorg.conf I use to make my tv an independant display: http://rafb.net/paste/results/etFPTa11.html

There may be some cards that are not supported by the nvidia driver. For example I had a Geforce2 GTS a few years ago and according to Nvidia (and their drivers) it was not possible to use vga and tvout at the same time, but with nvtv it was possible. I did not work very well, but it worked. But I bet there are other reasons why people prefer nvtv over the nvidia driver.
