I don't know when it started happening, but pulse started behaving like flat-volumes is enabled. My daemon.conf contains the line flat-volumes=no (it's mostly the default arch daemon.conf). I've tried removing /etc/pulse completely and reinstalling pulseaudio, but to no avail. Any ideas to what's happening here?

Define that a bit more. Is it happening with every application, only specific ones, do you notice a pattern? any files in your local .config or similar? What's the output of
pulseaudio --dump-conf
pactl list

Wow, I'm an idiot. Apparently I made a custom config file in .pulse a while ago, but only looked in .config/pulse while trying to fix this. Thanks a lot for bringing up the --dump-conf option!
