Missing menubar in application setting in KDE plasma

Hi,

I am trying to restore the in application menus for all apps in KDE plasma as per this screen shot "File Edit View Insert ..." currently I only have icons in each app.

I have googled answers for this and it seems to be related to putting the global menu icon back into the title bar. I can do this and get all the menus back but  not displayed as per the screen shot above, instead a breadcrumb navigation style menu. I want to have the menu bar appear across the top of the application. (As above)

I created a new user profile as a test that in effect is default KDE settings and I can hit CRTL+M in dolphin and the menu bar appears. This does not work in my main profile. CTRL M does nothing even though its set to this in keyboard shortcuts.

So looking at the screenshot below I have added the global icon in the red circle but I dont see the options for menubar style "in application"
I also deleted the ~/.kde4 folder in a hope to reset everything but it did nothing.
