Separate usr partition

On my system I use a separate partition for /usr and in general this works well.
At first I got the error that init could not be found since usr was not available at boot time but after adding the usr flag in mkinitcpio.conf the setup works fine in general.

The problem however is that sometimes, the error about init not being available reoccurs. Over all this happens mabe one in 50 boot times, somedays is happens 1 in 5 times.
There is nothing I do different when the error occurs, it appears to be compleately random.

Is there anything I can do to find out more about what causes this issue to resolve this?

Has anyone had similar issues?
