account locked after non-existent failed password attempts

Several times within the past few weeks, I noticed that xscreensaver reported to me that my account had been locked for 5 min due to 3 failed password attempts.  What is really odd about it is that I did not try and fail 3 times.  The state of the machine was as-if I did.

In searching, I found that others are experiencing something similar.  What would cause the false positive attempts?

Thank you for pointing me to the link, seth.  If I had my head screwed on straight this morning, I would have thought to search the bug tracker as well.

How many failed attempts were shown by the command `faillock`?
