I guess the boot partition is not big enough for the fallback image to be created.. I made a huge mistake back then during installation because it's just 100MB.
Is there a way to put the fallback image somewhere else or extend the partition?

you can try removing the fallback part of the preset or switch /boot to /boot/efi. you can also try to resize /boot but there's a high risk.

Thanks for the suggestions, in the end after a backup I wrote an install script and reinstalled the whole thing.
