What is a “smart scan”? Sounds like a marketing gibberish, not conveying any real meaning. Please explain in more detail what you mean. In terms of actual functionality.
The above question is meant to find a solution to your specific problem, not to give a direct answer. That’s because currently there is no open source AV other than ClamAV.

Here in the wiki you can find some anti virus/malware solutions. Regarding smart scan I have never heard about it so I am not sure what it is or what it could be.

That doesn’t explain much.
If by “startup items” you mean scanning your initramfs or systemd units: you can direct ClamAV to scan any file and directory you want, including those at “/boot” or ESP, and any files belonging to systemd. If by “memory” you mean opened/mapped files, see lsof. Be sure to read the “OUTPUT FOR OTHER PROGRAMS” section.
