I recently switched my father's box from Ubuntu to Arch. Using the same configuration files, I have been unable to restore functionality with his scroll wheel, or the other extra buttons. Xev will not display any output from any of the extra buttons, including the scroll wheel. It's as though it's only been recognised as a 3 button mouse.
It's a 9 button mouse from the "MS Optical Desktop Elite Bluetooth Keyboard and Mouse Combo" --> http://tinyurl.com/ywqpyj.
InputDevice section from Xorg.conf
Section "InputDevice" Identifier "Bluetooth Mouse" Driver "mouse" Option "CorePointer" Option "Buttons" "7" Option "Device" "/dev/input/mice" Option "Protocol" "ExplorerPS/2" Option "ZAxisMapping" "4 5" EndSection
Output from hciconfig -a
