Qt Designer is provided by qt (or qtmod)
pacman -Qo /usr/bin/designer
/usr/bin/designer appartient à qt 4.4.2-3Offline
/usr/bin/designer is owned by qt 4.4.2-3
Are you sure you are not just confused by the new name for the executable? (it's designer, not qtdesigner)

You get Qt designer when you download Qt; run /usr/bin/designer

I have no menu icon either -- using Gnome or XFCE. It seems to have disappeared at some point in the past months because I USED to have one.
