Mozilla Firefox has been flagged out of date for ages (?) and we are still running 0.91 while 0.99 was just released. How much work does it require to make a mantainance-update of the package?

I think 0.9.2 was only an update for the windows version, as it was that one that had the security hole.
And where were all the sportsmen who always pulled you though?

Firefox was not updated to 0.9.2 because it was just a windows bug fix, but it will be updated to 0.9.3 before "ages" occur.

Which, btw, works just fine. Copy /var/abs/network/mozilla-firefox to /var/abs/local (or wherever), change the PKGBUILD version entry to 0.9.3, makepkg, pacman -U mozilla-firefox-etcetcetc, done.
Takes quite a long time to compile, though. Works like a charm, mind.
