Twinkle and Gizmo, any success?

I'm trying to get Twinkle working with a Gizmo SIP account but I'm running into a problem.  When I make outgoing calls (and have credit to do so), even when calling a 1800 number I will get connected but hear *nothing* on my end.  But, if I dial the wrong number, I hear a message telling me that I've dialed a wrong number.  So, something funny is going on. 

If I dial anything within the Gizmo foo@proxy01.sipphone.com realm, I'm connected, but *everything is silent*.
If I dial an echo test service at some other realm (using the Gizmo account), such as sip:enum-echo-test@sip.nemox.net I can talk away and hear my voice transmitted back no problem.
If I use a Free World Dialup account I can also make calls without issue.

I'm stumped... does anyone have any idea why I'm experiencing this silence, only with Gizmo/Twinkle and the proxy01.sipphone.com domain?

Twinkle settings:
user name: 1747xxxxxxx
Domain: proxy01.sipphone.com
Authentication name: 1747xxxxxxx
Password: mypassword

STUN server: stun01.sipphone.com


Ports forwarded (TCP/UDP): 5004-5005, 64064
