create pdf form from image file

is there any way to create a pdf form from an image?  perhaps using ghostscript?  TIA

The only disadvantage of this utility is input image format is bmp only.

I believe imagemagick 'convert' will accomplish this, as well.

Sounds more like OCR would be a solution. What sort of image?

Install imagemagick to get this application.
