Simple one, how do I start a terminal program from a menu item?

It would depend on which wm or de you are using.

In any WM or DE, when you add an entry to the menu, you specify the command. So, just use the command to open the desired program in a new terminal window. For instance, to add "vim", since I use the "urxvt" terminal emulator...
urxvt -e vim
If you use terminator it would be "terminator -e vim", etc.

Well in openbox, you edit /home/whatever/.config/openbox/menu.xml, but because you haven't specified a WM or DE, I can't help

Sorry, It's KDE. I want to be able to open a command line DVORAK keyboard tutor in the terminal from the menu. I was writing it on my DVORAK keyboard, big mistake as it takes ages to write a post at the mo. Normally I do give a bit more info...

right/alternate click the menu icon in kde, then click menu editor. Then you can make your own shortcut

I am back home now so I can try it. Nice one austin.rbn, it was just that kind of feeding one command in to another I was after. In my case konsole -e dvorakng opens a console and starts the command line keyboard tutor.

you could still create a menu shortcut for that if you want. I have several like that. Example
I have vim set to open with xterm by a menu shortcut that I then placed on the bar for quick access
