Slow HD

Hi,
I'm having issues with one of my HDs, it is taking a long time to mount, originally I had a NTFS partition taking up the whole drive, but it started taking a long time to mount, and I ran CHKDSK from windows on it, which corrected some errors, so I had originally thought that was the issue. However that didn't fix it so I reformatted it and copied everything back over yet I'm still having issues accessing the drive: it takes a long time to mount, read and write. Does anyone have any suggestions on how to diagnose the problem, I'm fairly sure it's dying but I guess I need some proof if I want to try and claim a new one through warranty (it's only 2 years old).

I get this when running the extended test (it runs for about a second before throwing a read error up):

I used hparm to calculate read speed:
"hdparm -tT /dev/sdb" and got the following: Timing cached reads: 36.6kB/sec, Timing buffered disk reads: 2.95MB/sec
Time to claim a replacement under warranty I think.
