no more sound after update

Hi all,
I'm writing because I can not solve the following issue. I just updated the system but sound does not work anymore. I tried looking at wiki, but I still was not able to make it work.
the strange thing is that if i type "alsamixer" i don't see all controls but ONLY 1 related to pulse audio. I cannot understand why. If I then go to f6 and select my intel card, it is back with alsa controls. All of them are unmuted. Another very strange thing is that if I go through windows, I can not hear there anything for a while...randomly then i can hear again!! O_o

I tried testing aplay but no way to make it work.

I'm using Gnome3

First time I restarted system after update, i tried to restart alsa but I got:
/usr/sbin/alsactl: set_control:1464: Cannot write control '2:0:0:IEC958 Playback Default:0' : Operation not permitted
Now i ca not get any message again on alsa restart

I don't know what to check to verify sound card. I can post some of output that i tried to understand:

Then as root (because as user it gaves me many "not allowed")

Please show the output of this:

tail --lines=+$(grep -on Running /var/log/pacman.log | tail -2 | head -1 | awk -F':' '{print $1}') /var/log/pacman.log

It will show the logs from your last couple of pacman updates.

Please show the output of this:

tail --lines=+$(grep -on Running /var/log/pacman.log | tail -2 | head -1 | awk -F':' '{print $1}') /var/log/pacman.log

It will show the logs from your last couple of pacman updates.

Thanks for the answer. Just to be more complete I reported all the logs related to the update, in such a way to understand what happened...maybe. You'll see some errors with grep. I reinstalled grep and that error was no more there...i think:

I installed pavucontrol and this is the output while playing an mp3 file:

... more line like this...

I don't know if it is useful but it seems that when i play something i can hear some little noise...like a mosquito.

I think here here is the issue, this is my /etc/asound.conf:

# Use PulseAudio by default
pcm.!default {
  type pulse
  fallback "sysdefault"
  hint {
    show on
    description "Default ALSA Output (currently PulseAudio Sound Server)"
  }
}

ctl.!default {
  type pulse
  fallback "sysdefault"
}

# vim:set ft=alsaconf:

this means that pulseaudio is the default driver. Is it normal? I don't remember i set it as default. i tried to remove it but i realized that is a dependency of gnome (i'm useing gnome3). Is there anything wrong with that file? thanks

That is the same asound.conf that I have. I use pulseaudio. And when I run pavucontrol I get a window like this, even when pulseaudio itself is playing up.

is there a way to reset all alsa configuration? I even asked on the irc channel. they told me to try many things but the guy thinks there something missing/bad written in configuration and only was able to produce sound through headphones using aplay command

UPDATE:
Hi all, I just updated (alsa utils was updated!!) again and now I'm able to hear ONLY from headphones. no way (i know) to make it working speaker too.
Using gnome3 the only way is to choose from "sound settings" configuration menu: "Analog Stereo Output" or "Analog Stereo Duplex"
Any suggestion?
