For unknown reasons sometime in the past few weeks, my volume became nearly inaudible and no amount of tinkering with the pulseaudio settings helped, even when volume was turned fully up. Then I removed pulseaudio and working with Alsa I could see that the problem is PCM channel. Basically, it keeps resetting itself to 3% whenever volume is prompted in any way. A new track on the playlist in Amarok will re-set it, unplugging the power cable and switching to battery will re-set it and a ton of other events I haven't catalogued seem to be re-setting it as well when I am not looking.
I googled and found one advice somewhere that at least allowed me to change it up so it kept re-setting to max volume instead of close to mute, but that didn't survive a re-boot and I am now back to it re-setting to 3% PCM volume all the time.
And besides that, volume at 100% doesn't seem as high as it used to be.
Ideas? Suggestions?

That happened to me with Skype which configured by default to auto adjust volumes : Options > Sound devices > "allow skype to automatically adjust my mixer level".
Disabling that feature everything went back to normal.
