I will get the logs tonight. I'm guessing I should have a step between 3 and 4 where I actually replug the receiver?

I couldn't figure out the usbmon step (I mounted debugfs, but the usb directory only had a file named devices in it... I guess this is because I'm running off the installation disk).
However, I've got other logs.
This is when I boot with a wired USB keyboard and a unified receiver mouse. I have no idea whether the mouse is working, but my unified receiver keyboard (K800) didn't work at all.

I've had this problem as well after upgrading to 3.10. Finally I've found one USB port on which the unifying receiver works flawlessly. Though I have absolutely no idea why this one port does fine and the other five ports fail ...

@rainbowgoblin Those URLs are useless, I cannot access them with plowdown. Please use a better file hosting service in the future. The usbmon trace is really important, without that the dmesg and lsusb may be less useful.

If you do not see usbmon, try `modprobe usbmon` as root.
