Pulse Audio volume shoots to 100% on notification playback

Hi guys, i'm experiencing a strange problem. When I receive a notification (quassel-client/new mail/etc) my master volume shoots to 100%. You can immagine how annoying that is when listening to music or when in a conf call.

Any ideas on why this is happening and how to restore back to previous behaviour? (that is all volumes keep the settings i set and not what they decide).
Any info on how to debug this?

Could it be related to flat volumes (setting in daemon.conf)??
