rEFInd was recently upgraded. As usual, I made sure that my scripts had copied the new icons/, fonts/ and driver over, and I checked for changes in the .conf files. What I didn't notice initially is that icons/ now contains png files but the default refind.conf still references icns files. Rather oddly, rEFInd still finds icon files for entries for which I explicitly specify an icns file in my custom config file. However, it does not find icons for entries which are automatically generated by scanning for suitable boot entries.
This is not a big deal as it is merely cosmetic and the ugliness does not impair booting in any way. However, it seemed a little odd and I wondered if I'm somehow missed something such as a message telling me to do such-and-such? I don't think I've missed such a message but I would not like to guarantee it by any means.
EDIT: The latest version of rEFInd in stable solved this issue for me.
