T420 ALSA/Pulse: headphones aren't working

If I plug in headphones the sound gets muted but I do not get any sound. I have recently installed Pulse (rebooted since)

#load-module module-udev-detect
#load-module module-detect
load-module module-alsa-sink device=dmix
load-module module-alsa-source device=dsnoop

with this suggested change.

sudo hda-verb /dev/snd/hwC0D0 0x19 GET_PIN_SENSE 0 tells me the headphones are sensed correctly (0x1b is the mic in the same plug, that's sensed correctly too, headphone+mic triggers it, headphones only not).

pavucontrol shows the sound level rising and ebbing when I play music so that works; and after all Internal speakers work too.

I did amixer -c 0 set Headphone 100% already. What's next?

What codec/controller is that? Provide a datasheet link. Does the pin layout change in hdajackretask when plugged in/out? Can you get speaker-test to run through headphones?

After reboot I have ran alsamixer and noticed the headphones being muted. I am quite sure I haven't muted them. It's very weird that you can have volume levels separate from being muted .
