I'm using KDE 4.13.0
uname -a
Linux 3.14.1-1-ARCH #1 SMP PREEMPT Mon Apr 14 20:40:47 CEST 2014 x86_64 GNU/Linux
My soundcard is an onboard HDA Intel ALC1200.
I'm not using pulseaudio, just alsa and I'm using Gstreamer as the backend.
I generally have the master volume set to maximum and adjust the volume of individual apps either using their built in features or the dial on my speakers.
Within the last week or two the master volume keeps being reset to 50% maximum - without any intervention from me.
I don't believe I've changed anything to cause this change. In fact I don't even know how to do that.
For example yesterday I noticed it happened 3 times.
I haven't noticed any pattern to when it resets, or any behaviour from me to cause it.
This is quite annoying and I don't think it should be happening - I've tried searching but I can't even fnid anyone else that has reported the same thing.
Does anyone have any idea what can be causing this and how to stop it.

not sure if the same problem, but any KDE notification, test sound sets my Master to 100% volume. However, clementine audio player doesnt change it at all.
Allergic to admins&moderators. They scare me.

If your volume changes per application and/or gets reseted by notificacions, check these config files, and change flat-volume to no.
/etc/pulse/daemon.conf or ~/.config/pulse/daemon.conf
flat-volumes = no
I solved my problem editing /etc/pulse/daemon.conf
