In the pacman man page you can see this:
NoUpgrade = etc/passwd etc/group etc/shadow
NoUpgrade = etc/fstab
But in the default pacman.conf you can see this:
#NoUpgrade =
I'm sure, back in the day, the files shown in the man page were defaults. Now I gather pacman does NoUpgrade by default (maybe the last 10 years?)
Does the man page need updating or have I misunderstood the change?

It (the man page for pacman.conf, not pacman?) says "EXAMPLE". You can of course consider the default pacman.conf another example, but I see absolutely no reason why these two should be the same.
I don't know what you mean by "pacman does NoUpgrade by default". Packages can specify (config) files that need some extra attention in the backup array. Like NoUpgrade, that might result in pacman creating .pacnew files, but it is not the same.
