The "hot" pixel -- the pixel I think I am clicking on with the mouse -- sometimes is somewhere else than it should be. I suspect it is the correct pixel, but for a different mouse pointer than currently displayed.
It's highly annoying because it's hard to select anything! Generally the actual hot pixel is about one cursor position to the left and to the top from where mouse pointer's hot pixel appears to be.
Worse, it happens on and off. I cannot think of anything I am doing that causes the change from correct to offsetted, and back.
This is KDE Plasma on VirtualBox on the Mac. The problem occurs independent of application.
Any ideas?

I have seen the problem with an Arch guest with openbox running in a VirtualBox VM on a Windows 8.1 host.
The common criterion seems to be VirtualBox. Are you running full screen, or in a window?

Playing with the various VirtualBox "mode" settings: full-screen vs. scaled vs seamless etc it does seem to impact whether offset happens or not, but not predictably as far as I can tell. Perhaps the unpredictability comes from screen savers coming on, or things like that, as result of which VirtualBox does some kind of re-initialization. Speculating, of course ...
