When I ran (from the examples)... .. .
reflector --verbose --country 'United States' -l 200 -p http --sort rate --save /etc/pacman.d/mirrorlist
it returned... .. .

Actually, this may be correct. If you look at the mirror status page, all of the other US mirrors have a very low completion percentage right now, I'm sure due to some moving around of servers internally.
And please edit your bug link so that it doesn't automatically vote for the ticket in question.

And more info, it appears that the completion percentage is how often a mirror status check has completed over a period of time (24hrs? not sure). The status checks failed for a while as things were moved between servers. Once the time period has passed, everything will be back to normal.

You can specify -c/--country multiple times on the reflector command line. Useful for getting a wider range of available mirrors. I personally use
reflector -c "United Kingdom" -c France -c Netherlands -l 40 -f 3 ...
as the French and Dutch mirrors were equally fast for me when I did some tests.
