PCI Passthrough: VM Crashes as soon as I start it

The title basically says it all.  I am running a system with Intel i7-960 and VT-d is enabled within the BIOS settings.  IOMMU groups show up fine, as shown here:

I am using the 5450 as my main GPU for linux (it is plugged into the first slot, with my monitor hooked up to it) and hoping to pass the RX 480 to my Windows VM, and I followed the guide on the wiki the best I could.  Basically everything is set up as described (except, of course I'm using BIOS rather than EFI, but it should still work).
My problem is this:  whenever the PCI is set within the devices on the XML file, or whenever I add them using the virt-manager GUI, and then run the virtual machine, it crashes immediately.  I don't know what happens, but when I try using virsh on a terminal it does the same thing also.  No errors show up in the terminal, though, so I don't even know what's going on.
