I would expect libvirt/virt-manager to pass -cpu host to qemu , the cpu section of libvirt xml files is tricky to get right though .
(and I can't help much with that , i prefer qemu commandline )

As for kvm=off , i've only seen that used in a PCI passthrough setup .
It seems Nvidia doesn't want consumer hardware to run at full capacity in a VM, combining PCI Passthrough of a Nvidia card with kvm=off is a workaround for that.
