I have a freshly installed Arch with KDE (plasma-desktop pkg) and I don't have sound in KDE application. Mainly the notifications sound. But there is sound in chromium, firefox, GTK+ apps, deadbeef(pulse output) and mpv(pulse output).
There are no cards except Default in System Settings - Multimedia - Device Preference. The Test button doesn't work either.
On the Audio Hardware Setup tab, the Build-in Audio is always selected and I can't choose my own card (it being reset after System Setting window restart).
I'm using the Focusrite 2i2 USB sound card with the GStreamer Phonon backend.
I've tried with and without pulseaudio.
I have Gentoo installed in parallel (no systemd/pulseaudio) and sound works in KDE apps. So it's not a hardware problem.

Install relevant gstreamer plugins, primarily for phonon's pulse support it is useful to have
gst-plugins-good
installed, more plugins for more codecs wouldn't hurt either, but specifically for pulseaudio integration gst-plugins-good is required.

Thanks. After installing gst-plugins-good the devices appear in System Settings, but still no sound.

That looks somewhat alright apart from the corked and idle states on the sink. FWIW if you've just installed the gstreamer plugins you might have to restart plasma for full effect. Otherwise, to rule out corking issues comment/remove
load-module module-role-cork
from your /etc/default.pa are you running some VOIP client like teamspeaker/mumble or similar in the background?

Commented module-role-cork out - no effect.
Also, after pressing the Test button in System Setting's Multimedia tab there are no changes in pulseaudio applications list. So it probably doesn't use pulse at all.

It should be using it, and at least the dolphin invocation you used to test does register on the pulse server. You did run the fuser command as root? Can you repost these outputs during playback of something that works?
