Just installed my old favorite adapta-gtk-theme on Gnome Shell and noticed some inconsistencies with the theme.

And when you post images, give some context. All I see is three screenshots with different themes applied. What are we supposed to notice? What is inconsistent? Is one of these images what you want and another what you have?

There was a new Adapta release 3.95.0 which basically was a complete redisign of the Adapta theme (some say a new theme). There was much controversy about this, so upstream reverted those changed and released 3.95.0.11, which basically is just the old 3.94 version re-released.
On Arch the adapta-gtk-theme package is marked out-of-date, but not updated yet. There is likely a reason
As it stands currently the adapta theme itself is unmaintained. There were plans to move the changed code into a new repository under a new name, but I don't know what happened to this.
There was a discussion on the Github issue tracker about this whole situation, but they turned the issue tracking of on https://github.com/adapta-project/adapta-gtk-theme/
