Why are some packages shipping systemd service file(s)?

Most upstream packages are already providing a systemd service file, wouldn't it be better to simple patch the service if custom things are needed or extend the service by a sub-unit?

Arch typically ships upstream defaults unless there is a damn good reason not to, what "custom things" are you referring to? Please post examples.
