Whenever an application (e.g. firefox, keepassx2, vscode) sends something to the clipboard, the first time the output paste is empty. It only works if I hit Ctrl+C twice (or y for vscode vim plugin). I'm using plasma as my desktop environment. No klipper running.

When did it start for you?
I have exactly the same problem. For me, it's a fresh install of Arch on a brand new computer. The other wrinkle is that I'm using bspwm as my window manager. And I remember playing with the settings for the KDE's included clipboard history programming running in the system tray.
I found a post explaining how to disable it (https://bugs.kde.org/show_bug.cgi?id=352171#c15), and so far it seems to be helping. I'm not totally sure if that's the fix or not. For me, I didn't want the clipboard tool running anyway, so hopefully this works out for me. I'll let you know if I'm still having the issue where I have hit copy twice.

I actively use and like the clipboard and I don't have this issue. Do you maybe have multiple panels/clipboard managers?
