I have an onboard sound card (00:1b.0 Audio device: Intel Corporation 5 Series/3400 Series Chipset High Definition Audio (rev 05)) whose SPDIF output worked in Windows.
I have tried https://wiki.archlinux.org/index.php/Pu … n_the_card which, in spite of the card name being correct, gives me:

When I went to test out spdif on my mobo I needed to go into alsamixer (alsamixer -c 0) and unmute one of the spdif options there (m).

Which is normal, an SPDIF doesn't have a volume but you did unmute it. From what we see here your spdif device is allocated as a completely separate playback device, it's possible that that case isn't yet properly handled by pulse (... or it got broken by the too new pipewire udev config...). Do you get output if you do something like
speaker-test -c2 -Dhw:0,3
? If that works, a bit of a hacky way you could add to a pulse conf is allocating a hardcoded sink to the respective device with something like
load-module module-alsa-sink sink_name="SPDIF Output" device=3 device_id=0
