How to reproduce:
- After booting the sound from my laptop's build-in speakers works fine.
- Plug in headphones; Sound comes through the headphones
- Unplug headphones; Sound output switches back to speakers, but no sound is coming out of them.
As a workaround it's possible to reset it by putting the laptop to sleep with the headphones unplugged (or unplug the headphone when the laptop is already sleeping).

With headphones plugged in:
pacmd list-sinks
amixer -c0
With headphones unplugged:
pacmd list-sinks
amixer -c0
Note that the speaker's availability is now "unknown" instead of "no".
Similar to: https://bbs.archlinux.org/viewtopic.php?id=262673 only here it switches to the right sink, but the speakers still don't work.
EDIT:
I had only one line difference with the default pulseaudio config, but I removed that to see if that was the problem. I temporarily fixed the problem for about 3 unplugs and after that the issue was back.
default-sample-rate = 48000
So even the default pulseaudio configuration has the same problem.
