This was working fine, and suddenly stopped working.
I'm using a LGCX48 as my main monitor (where the sound should be coming from).
As I said it was working fine, and after a couple of reboots stopped working. I have completely shutdown my computer, unplugged power to both my monitor and system. Unplugged the HDMI (while off) and plugged it all back in. Still nothing. I also went into the settings on the display and tried toggling the HDMI sound mode from bitstream to PCM and rebooted each time, but to no avail.
In Pavucontrol > Configuration > High Definition Audio Controller > Profile there are 6 Digital Stereo (HDMI) options. I have tried each one (only 3 are listed as being plugged in) - my other 2 monitors have speakers as well and they are enabled, but audio does not get piped to those either. I tried setting them to the Digital Surround 7.1/5.1 (HDMI) and that promptly crashed Pavucontrol, and would not let me open it again. I had to use pactl to set the profile again.
I can post more outputs if necessary.

Kind of solved, not sure what the problem is/was, but switching the monitor to 60hz allowed audio to play, and audio continues to play when switched back to 120hz. Haven't tested whether it will persist through a reboot, but that's easy enough to script. Could be something with autorandr vs the nvidia-settings application. I will do some further investigating and see if I can find what is causing the issue.
