Nvidia uses the snd-hda-intel driver for the audio backend, none of your outputs check for that.
aplay -lL
sudo dmesg | grep -iE 'snd|hda'
pactl list cards

Something appears to be very wrong in your setup.
Please post full lspci -k output as well as full dmesg and journal outputs from one boot.
Disliking systemd intensely, but not satisfied with alternatives so focusing on taming systemd.

You most likely have a botched /etc/asound.conf or ~/.asoundrc post either or both of them if existing, in doubt install strace post
strace aplay -l
or a wrong modprobe config, post that as well if you have anything trying to manipulate audio

strace aplay has lots of

Your bootcommand line mentions many lsm modules .
[ 0.000000] Command line: initrd=\amd-ucode.img initrd=\initramfs-linux.img root=PARTUUID=153e1f20-7e7b-492d-a965-c0682fc61c2c zswap.enabled=0 rw intel_pstate=no_hwp rootfstype=ext4 systemd.unified_cgroup_hierarchy=false lsm=landlock,lockdown,yama,integrity,apparmor,bpf nvidia_drm.modeset=1
The journal shows you're running apparmor.
I know next to nothing about apparmor, but suggest you check whether the profiles you use with it are blocking access to sound devices.

Disliking systemd intensely, but not satisfied with alternatives so focusing on taming systemd.
(A works at time B) && (time C > time B ) ≠ (A works at time C)

I've remove lsm=landlock, lockdown, yama, integrity, apparmor, bpf from kernel parameters and disabled apparmor profiles.
Although the issue remains.

recently there have been reports of hdmi related issues with nvidia driver 515.76 , https://bugs.archlinux.org/task/75995
Can you boot with another monitor that doesn't use hdmi to see if sounds works then ?
Incase you don't have another monitor, a workaround that helped some was to boot without the hdmi monitor plugged in and plug it in after the system has started up
