xrandr (randomly) stops being able to set the resolution

Setup: I have a USB-C -> HDMI adapter which connects my laptop (Dell XPS 9320) to my external monitor (Dell U2719D). I use autorandr to make sure I have the right screens setup each time.

Up until recently, this worked flawlessly every time, but in the last month or so, at random the external monitor doesn't get assigned properly, and remains blank with "no HDMI signal from your device".

On digging into this, I have found that sometimes the 2560x1440_59.95 mode just isn't present on the DP-1 monitor, so trying to do:

xrandr --output DP-1 --right-of eDP-1 --primary --auto

Gives me a weird, stretched resolution.

Following the Wiki (and several SO questions), I tried to add this mode manually. First I used `gtf` to find the modeline:

Which worked fine. Finally, I tried to set this resolution, and I get this mysterious error which none of the posts I have seen have helped with:

Even if I try to specify `--crtc 0` I get the same error as before.

Previously, unplugging the monitor and plugging back in has worked, but now it isn't (I have tried five times so far).

so your generic modelines are wrong (edit: polarity!)

Since the edid still seems to advertise the mode, I'd look into the xorg log whether and why it's filtered.

You don't want to add the mode at all, the EDID advertises it. If the server rejects it from the edid, it'll reject if from xrandr as well.
Did you inspect the Xorg log?

Ah ok this is super interesting! I have two: one is an Anker USB-C hub with an HDMI port (my laptop only has USB-C) and the other is an HDMI cable connecting to the monitor from that Anker hub.

I had a similar problem just now: booted fine, external monitor working fine (X not yet started), login, then startx and screen goes black, "no signal from your device". Unplug, replug, and it works. Full log below. Things I'm struck by, however, are these lines.

I guess the way this works is that the monitor is permanently connected to the hub and you're plugging in the usb-c cable when you want to use it?
And that usb-c cable is probably the hubs only power supply?

Has/does this ever happen/ed when you boot the system when it's already plugged into the  hub?

Not quite: hub has a power supply (95W in) and is mounted on the desk. Whenever laptop is at the desk (~75% of the time) it is plugged into the hub which also has a load of other stuff (external HD, keyboard, mouse, webcam).

The most recent occurrence, with logs above, is from when the laptop booted when connected to the hub, which worked fine until the X server started.

Once the monitor is working, it never randomly stops working, I only have this problem on connecting.
