They are from the firefox theme "iPox". I just copied the Toolbar images from iPox into MidnightFox by the same author. (I couldn't stand the tacky buttons.)
The images, unfortunately, aren't available as separate images... they are in one big image file and I guess the theme extracts the correct portion of the image for each button.
Edit: Since i already had the theme extracted... I went ahead and uploaded the image here.

I couldn't stay out of this thread any longer.
Since our little ssinfo thing doesn't work with sawfish here is the pertinent info:
wm: sawfish
wm theme: Origami
gtk theme: Clearlooks-Grey (I hacked it from the original)
Icons: Tango
gtk font: standard issue alias Sans 8 (urxvt font is terminus)
wallpaper: our good old ArchBlue ^__~
I'm a minimal kind of guy, so it's mostly a screenshot of conky ^^

Simply by activating the beryl video plugin (look in beryl-settings)
You will have a file.seom
Play :
seom-player /tmp/beryl-capture.seom
Encode :
seom-filter /tmp/beryl-capture.seom | mencoder - -ovc xvid -xvidencopts bitrate=1200 -o /tmp/beryl-capture.avi
