AMDGPU crashing while gaming & PC stuck on blank screen when poweroff

#AMDGPU CRASHING:
Running Gigabyte Eagle RX 6700 XT (Non-OC).

So I suffer from an AMDGPU issue for a pretty long time right now. I believe it's been there since April and for now it kicks in only while playing ACValhalla. This is very random and difficult to reproduce. Sometimes I can play for 3-4 days without any crash and sometimes I can just play the game for 20 minutes since launch and it crashes immediately leaving my screen black and PC unresponsive forcing me to use REISUB method for safe reboot. I use the GPU mostly with undervolting applied tho I managed to reproduce the issue both with undervolting applied and default settings. Also I have done every GPU test both with default and undervolt.

This log shows that it's mostly ACValhalla that triggers it somehow but you can see there also "kwin_wayland" process doing that. I'm pretty sure I was experimenting with GPU Reset modes and that could trigger the issue by an accident. I don't remember any other task than gaming where the issue happened.
It seems like my GPU is struggling with all reset modes. Mode0, Mode1 and Mode2 are producing the same result while only BACO shows a slight difference yet still not what is probably desired. So there is my attempt to reset the GPU in Mode2 and BACO:

I described the result with "###" at the end of each reset.


I've done multiple tests of my GPU, including: Memtest_vulkan (once for 3 hours, second time for 6 hours - both passed with no errors), Furmark VK & GL, Unigine Superposition Benchmark, Unigine Heaven Benchmark, bunch of in-game benchmarks (CP 2077, AC:Valhalla, AC:Origins, AC:Odyssey).

What's strange is that in Furmark OpenGL everything runs smooth but when I launch Furmark Vulkan strange things happen on my monitor:

I did not experience this issue anywhere else, only Furmark Vulkan.

Solution:
While using RADV keep your window focused, do not click other windows otherwise it will produce the artifacts.
Either way, switch to AMDVLK.

I was switching between kernels (linux, linux-lts, self-compiled TkG kernel), I removed xf86-video-amdgpu and the issue was still reproducing randomly in ACValhalla. I've seen many posts in the internet with similar issue and I still didn't manage to fix it. I'm also not really sure if this is software related only or hardware related and if I should RMA the GPU.

#PC STUCK ON BLANK SCREEN WHEN POWEROFF:
So this is kinda strange and there I have totally no idea what could cause that. When I suspend, reboot or shutdown the PC sometimes gets stuck in the moment when monitors, keyboard and mouse are getting disconnected (even HDD makes the parking sound) and the PC is supposed to shut itself down but instead it stays ON forever until I hard shutdown it.
Today it happened while shutting down, and there is the log of it:

UPDATE: I've done some troubleshooting related to the strange artifacts in furmark. This is 100% software related.
RADV vulkan driver produces these strange artifacts while you are running the benchmark and unfocus the furmark's window for example by clicking on firefox's window. If you click furmark's window back it reclaims focus and artifacts disappear.

AMDVLK does not reproduce this issue and benchmark runs smooth as butter.

In terms of crashing:
It crashes under RADV, AMDVLK and even AMDGPU-PRO with the same output. Regardless what I've changed in my system, ACValhalla kept crashing my driver over and over again with longer or shorter gaps.

I didn't reproduce the issue in other games so I guess it may be a software issue tho It also may be a good idea to just RMA it for the piece of mind.
