You tell Xorg to load the proprietary nvidia module, but Xorg loads the open source nv-module. Did you remember to pacman the nvidia-package?
I'd remove all those modeline-, HorizSync- and VertRefresh-lines in the Monitor section aswell and let Xorg figure out the correct values itself.
