I'm not sure what the problem is but from what I can tell its probably due to Xinerama. I can't replicate this bug consistently, it seems to only appear after Xorg has been running for several days.
Running everything latest from repo.
Running on dual opteron, with 6gb ram, 64-bit arch, 2x7800GTX nvidia's, 4 screens attached, and i'm using the libfakexinerama patch
Some outputs:

Any ideas why this might be happening?

try reinstalling nvidia driver and nvidia-utils, hopefully it will solve the libglx symlink at /usr/lib/xorg/modules/extensions/libglx.so

Reinstalled it but still having crashes.

There is no xorg package in Arch, as well as Arch's nVidia driver is newer. The output you have is unlike anything that I've ever seen. This is Arch?
Offline
Yup This is arch.
Did a pacman -syu today
[geniesis@skynet ~]$ pacman -Qi xorg
Name : xorg
Version : 11R7.0-1
Yes, i know its a dummy package. But it references the versions.
[geniesis@skynet ~]$ pacman -Qi xorg-server
Name : xorg-server
Version : 1.5.3-4
[geniesis@skynet ~]$ pacman -Qi nvidia
Name : nvidia
Version : 180.22-1
[geniesis@skynet ~]$ pacman -Qi nvidia-utils
Name : nvidia-utils
Version : 180.22-1
[geniesis@skynet ~]$ uname -a
Linux skynet 2.6.28-ARCH #1 SMP PREEMPT Sun Jan 25 09:43:53 UTC 2009 x86_64 Dual Core AMD Opteron(tm) Processor 280 AuthenticAMD GNU/Linux
Its still happening. And its the same error as before.

There is no xorg package in arch! And the newest version is 7.4, so you need to remove that package and install xorg group packages again. Like this:
pacman -S xorg
and you'll get this packages installed:
xf86-input-keyboard xf86-input-mouse xf86-video-vesa xorg-fonts-100dpi
xorg-fonts-75dpi xorg-res-utils xorg-server xorg-twm xorg-xinit xterm

Crashed again today with the same error.
This is after i tried ahcaliskan's recommendation of doing pacman -S xorg.

did you remove the old "xorg" package first?
Offline
Yup,
But still crashing

I am having the same issue and I believe it all started after I updated xorg-server.
My last crash produced the following.

What version of xorg-server are you running?
> yaourt -Qs xorg-server
I am running:
extra/xorg-server 1.5.3-4 (xorg)
X.Org X servers
extra/xorg-server-utils 7.4-2
X.Org utilities required by xorg-serv

Please disregard what i have said in my previous post. I have found the solution to my problem.
Recently I upgraded to 8GB of ram which is when the problems started appearing.
Everyhting was segfaulting; firefox, xorg, virtualbox, etc. Really unstable system with random crashes and freezes along with segmentation faults like no ones business.
I thought it had to do with recent updates I installed, but after much research I found the problem lay within the DDR2 Ram Voltage. My Gigabyte P35-DS3L motherboard has a standard ram voltage of 1.8V (most MOBO's do), but the 8GB's of OCZ ram I upgraded to require 2.2V. Running it on auto (1.8V) was causing all the problems, so, after I overvolted by +0.5v in my BIOS, approximatly 2.3V, it is finally running stable again! Yay for happy Arch!
Anyways, thats may not be your prob gen2, but check into it... and I hope this helps others in a similar situation.
