I'm not sure about what would cause this, but my advise would be to use UUID.
Using these unique identifiers, this should not happen again.

Did you plug in some USB drive-stick?
With UUID you will have no problems with sd* labels.

Well, this has been a known kernel issue for years. The Persistent block device naming article covers UUID as well as other methods to workaround. Why this happens, or why it will not be fixed is beyond me. My best guess is that it's a rare issue. (?)

This happened to me as well with the latest kernel update.
I used a Live CD, mounted the root and boot partitions, edited /etc/fstab and /boot/grub/menu.lst then rebooted

Many thanks
I used id and uuid solved this .brazzmonkey and Misfit138 's advice quite wondful.

My system loves to change the order of drives randomly every time I reboot, so I've just switched to UUIDs. I think it looks rather ugly, but it solves the issue.

In the wiki article Misfit138 referred to they explain the use of partition labels too, this is _much_ cleaner than the obfuscated UUIDs and evenly robust for device node changes.

That's what I use.

I have the same issue. With UUIDs I am OK, but is there any chance this can be the cause of not being able to resume after suspend to RAM? To be more specific, after resuming I get tons of filesystem errors on the screen and I am forced to hard reboot.

A side issue with this UUID vs LABEL.
What effect on mdadm raid devices will the use of LABEL make since the mkinitcpio entry into initramfs is UUID with the present mdadm setup.?
Does mdadm use LABEL as well?
