I don't rotate my screen to often, so it's hard for me to be sure, but I'm fairly certain that it was after I installed the latest nvidia drivers that screen rotation no longer works.
When I try xrandr -o left or I try to rotate within nvidia-settings, I get this:
X Error of failed request: BadValue (integer parameter out of range for operation)
Major opcode of failed request: 154 (RANDR)
Minor opcode of failed request: 2 (RRSetScreenConfig)
Value in failed request: 0x780
Serial number of failed request: 14
Current serial number in output stream: 14
When I more explicitly write out, xrandr --output default --rotate left, I get this:
xrandr: Failed to get size of gamma for output default
xrandr: screen cannot be larger than 1920x1200 (desired size 1200x1920)
If I lower my resolution so that both sides are less then 1200, then I can freely rotate.
Any thoughts?

What's the size of the virtual screen (or whatever the name is) in your xorg.conf or some other config file?
IIRC it should go into the "Screen" section:
# ADD A VIRTUAL LINE TO PROVIDE FOR THE LARGEST SCREENS YOU WILL HOTPLUG
Virtual 2048 2048

There doesn't appear to be any resolution information in my xorg.conf
I'll test out the virtual thingy now...

I had the same problem. Downgrading xorg-server (and xorg-server-common) from 1.9.3 to 1.9.2-2 fixed it for me.

Please report it on flyspray. Or perhaps check whether the same occurs on other distros (nvidia forums looks like a good place to start)
