That layout is known as "(@) spiral". I use dwm-sprinkles from AUR which contains a lot of different patches. You might want to take PKGBUILD from that AUR link and modify "config.def.h" file and compile it to your liking.

Also jasonwryan is right about that one, so you just might want to patch your dwm alone.

QtCurve and Colors
Tile Bar
System and Conky Fonts
Icons
Wallpaper: Easy to find; I spent almost an hour on Google trying to find the original artist, to no avail.  There are variations of it floating around, too.

Sure thing.  I'm actually new to MPD in general (just set this up four days ago), so you might have to clean this up a bit; I'm still tweaking it myself, trying to get the track switch/crossfade to work the way I'd like it to.
